Important Announcements H1N1 (Swine Flu) infoBy now everyone has been hearing about the H1N1 (Swine Flu). Here at L-P we are monitoring students that are reported ill by a parent or sent home through the nurse's office. You may receive a phone call if your student is out ill 2 or more days in a row to assess for flu-like symptoms.
If your student is seen by the nurse and has a fever of >100.0 you will be contacted to pick your student up. Please make sure you have phone numbers where you are able to be reached. Please visit the LaSalle Co. Health Dept. and Illinois Health Dept. websites (links on this page) to stay up to date with current information. Thank you for your cooperation! Don't share items that you put in your mouth (straws, spoons, forks, cups, etc) STAY HOME if you are ill with a
fever greater than 100.0 COVER YOUR COUGH WASH YOUR HANDS OR USE HAND SANITIZERS IF YOUR STUDENT IS OUT WITH A FEVER > 100.0, REGARDLESS OF THE ORIGIN, THEY MUST BE FEVER FREE FOR 24 HOURS WITHOUT USE OF FEVER REDUCERS BEFORE RETURNING TO SCHOOL ANY QUESTIONS, PLEASE CALL THE NURSE CLASS OF 20149th grade physicals are required by law.
A sports physical will NOT be required for the 9th grade because it is included on the Illinois State Health Exam form. Please be aware that it is good for 1 year only! That means if your student plays a spring sport you will want to schedule the physical for June or July. A sports physical will NOT take the place of a 9th grade physical. DON'T FORGET TO HAVE A NEW MEDICATION FORM COMPLETED FOR EACH SCHOOL YEAR!!Don't forget that a physician order and parent signature on a MEDICATION FORM is required before medication can be given in school. A new MEDICATION FORM must be completed each school year.
MEDICATION FORMS can be printed from the FILES & FORMS or by visiting the nurse's office at school. Medication can not be carried by a student without a MEDICATION FORM on file for the current school year. The nurse will NOT give any over-the-counter medication (Tylenol, Motrin, antacids, etc) without a MEDICATION FORM completed. Files & Forms
